Trauma-Informed Expressive Arts Therapy is a specific approach to traumatic stress developed by Dr. Cathy Malchiodi that integrates "brain-wise" arts-based interventions and embodied awareness concepts to support trauma reparation and recovery. Expressive arts therapy [the combined use of art, music/sound, dance/movement, enactment/improvisation, storytelling/narrative, play, and imagination] is the key psychotherapeutic strategy when addressing traumatic stress. It also integrates current best practices in trauma-informed care with what is known about how the expressive arts and play assist in trauma reparation and integration.

The Seven Core Concepts of Trauma-Informed Expressive Arts Therapy [from © 2020 C. Malchiodi Trauma and Expressive Arts Therapy: Brain, Body, and Imagination in the Healing Process]:

1. Neurodevelopment and neurobiology inform the application of expressive arts therapy to trauma-informed intervention. As previously stated, trauma is not just a psychological experience; it is also mind-body experience. The role of neurodevelopment and neurobiology are central to using the expressive arts to address trauma reactions and to assist individuals in reconnecting implicit (sensory) and explicit (declarative) memories of trauma. In particular, neurodevelopment provides a framework for determining how to apply expressive arts interventions to various goals of treatment, including when and how to support self-regulation and self-efficacy, positive attachment, and resilience-building. 

2. Expressive arts therapy is focused on supporting self-regulation and co-regulation. Over-activation, hyperarousal, general anxiety are common manifestations of not only posttraumatic stress, but also other trauma-related challenges. Expressive arts interventions are used to not only support individuals’ own internal resources, but also provide various creative, action-oriented approaches to self-regulation and co-regulation when applied within groups.

3. Expressive arts therapy is used to help identify and ameliorate the body’s experience of distress. Individuals who are experiencing trauma-related reactions typically experience the impact of these reactions not only in altered thinking, but also various somatic experiences. Because the expressive arts are “embodied” experiences, they are helpful in identifying and repairing the body’s responses to trauma.  In particular, key trauma-informed practices are: using expressive arts to support individuals’ bodies as resources and normalizing the body’s reactions to trauma as adaptive coping rather than pathology, 

4. Expressive arts therapy is used to establish and support a sense of safety, positive attachment and prosocial relationships. Reconnecting with a sense of safety is central to trauma-informed practice. In particular, expressive arts approaches are used to help individuals recover a sense of well-being internally and in relationships with others. This also includes providing various opportunities for the individual to engage in creative experimentation that integrates experiences of unconditional appreciation, guidance, and support, experiences found in families with secure attachment relationships. When applied as group interventions, expressive arts support prosocial interactions and connect individuals through community.

5. Expressive arts therapy is used to support strengths and enhance resilience. Trauma-informed practice encourages helping professionals to see all individuals as capable of growth and reparation. It also holds the concept of resiliency as central to recovery. Expressive arts interventions are life-affirming and honor individuals’ capacity for resilience and personal strength by encouraging mastery with a goal of moving individuals’ self-perceptions from victim to survivor to “thriver”.

6. Expressive arts therapy respects the individual’s preferences for self-expression, particularly of trauma narratives. Trauma-informed practice emphasizes the role of individuals in their own treatment and their preferences for participation; these preferences are determined by culture, previous experiences, worldviews, values, and other dynamics. Arts-based approaches offer a variety of ways for expressing “what happened” dependent on the individual’s comfort level with self-expression. These therapies also respect the use of personal metaphors and symbols that allow individuals to control how they communicate sensitive experiences.

7. Expressive arts therapy provides meaning-making experiences and ways to imagine new narratives, post-trauma. As previously stated, expressive arts in particular allow individuals to convey what is often unspeakable. They also allow survivors to explore, restructure, reframe and re-story trauma and loss through non-verbal, asset-driven, participatory and self-empowering ways.

Resilience, Posttraumatic Growth and Trauma-Informed Expressive Arts Therapy Practices (Part Two) --building on the concepts in Part One, this course introduces the importance of the practices of gratitude and compassion, including self-compassion, in trauma integration and recovery. The emphasis is on using arts therapies and creative interventions to help individuals of all ages explore positivity as a wellness factor and approach to mediating trauma reactions. Additional trauma-informed practices including mindfulness and mind-body approaches are introduced to demonstrate their integration with arts therapies principles.Provides 6 hours of continuing education.

Expressive Arts Therapy and Positive Psychology-- The course covers 1) basic principles of positive psychology; 2) the interface and relationship between positive psychology, expressive art therapy and creativity; 3) neurobiology of positive psychology; 4) the concept of flow and its importance to positivity; 5) compassion, connection, courage and contribution as factors in positivity and wellness; and 6) art-based activities related to the theme of positive psychology. All readings and resources are available online; you will also engage in several hands-on activities, short feedback exercises, and complete a short final exam. Provides 6 hours of continuing education.

Ethics of Art and Play Therapy with Traumatized Children- an introduction to the ethical and legal issues of applying art therapy and play therapy to work with children who have experienced traumatic events, including interpersonal violence, witness to violence, hospitalization, divorce and custody disputes, and disasters. A trauma-informed approach to solving ethical dilemmas and forensic evaluations is included, with an emphasis on questions about retention of artwork, storage and confidentiality. Fulfills most requirements for 6 hours of ethics coursework and particularly helpful for those who are not credentialed as art or play therapists. Provides 6 hours of continuing education.

Trauma-Informed Expressive Arts Therapy with Children and Families--This course is an overview of art therapy, play therapy and creative arts therapies [music therapy, drama therapy and bibliotherapy] in trauma-informed practice with children and families. Course takers will learn why these approaches are essential to trauma intervention with children and families through short response papers and hands-on experience. Creative interventions allow children and families to actively imagine, experiment with, reframe and rehearse desired changes through self-expression; in this course you will learn a wide variety ways to facilitate trauma-informed, creative interventions and learn more about the foundations of these approaches. Provides 6 hours of continuing education.

Body-Based Approaches and Expressive Arts Therapy-- Focusing on the physical practices of yoga, this course will bring to light the "how" and "why" utilizing a body-based method is important when addressing trauma. One will both learn and experience these concepts though a variety of articles, films, and research, all provided within this course. Yoga is presented as a body-based approach to building resilience and self-regulation with consideration for trauma-informed methods of counseling and expressive arts therapy. Provides 6 hours of continuing education.

Expressive Arts Therapy Approaches to Self-Regulation and Stress Reduction. This course designed to help you understand and learn more about how expressive arts therapy approaches and methods complement and enhance psychotherapy and counseling. This course emphasizes 1) what is self-regulation and why is it key to trauma-informed intervention 2) current knowledge about self-regulation and stress reduction in psychotherapy and counseling; 3) the neurobiology of self-regulation, including polyvagal theory, interoception, neuroception and social engagement as applied to expressive arts therapy and trauma-informed practice; and 4) a variety of expressive arts therapy strategies for application in psychotherapy and counseling.

Introduction to Expressive Arts Therapy-- This course provides an overview and foundation for understanding the theory and practice of expressive arts therapy in mental health, counseling, psychosocial intervention and overall wellness. It also serves as part of the foundational learning for those accumulating credit hours toward Registered Expressive Arts Therapist credential with the International Expressive Arts Therapy Association. Learners will complete a series of short written assignments, quizzes, and hands-on activities to learn more about expressive arts therapy, history of the field and the essential concepts involved in applying approaches to counseling, psychotherapy and arts-based facilitation with children, adolescents, and adults. This course is open to mental health and healthcare professions, expressive arts facilitators, and graduate students who want to learn more about expressive arts therapies and creative interventions. Expressive Arts Therapy | Contemporary Approaches to Practice-- This course is part of the foundational learning in the Expressive Arts Therapies with a focus on major contemporary approaches to practice-- Appalachian Approach, Person-Centered Expressive Arts, Trauma-Informed Expressive Arts Therapy, Social Action and Focusing-Oriented Expressive Arts. I was first introduced to trauma therapy as a student and client trying to live with my own struggles as a trauma survivor. My journey continues as a clinician, scholar, and teacher of sustainable trauma integration therapy.   From many years of observing and working on my own dynamics and working with clients on four continents, I've come to see how important it is to view mental health symptoms in the context of underlying root causes.  


Trauma, caused by emotional injury or cognitive, physical, spiritual or social harm, is a root cause of problems for a surprisingly large percentage of clients with mental health symptoms. 

Clinicians may be technically correct in diagnosing such clients as having Depression, Anxiety, Bipolar disorder, personality disorders, RAD, Addictions, Eating Disorders and so forth.   But when trauma lies at the core of symptoms, such a  diagnosis can harm more than it helps.   Trauma integration is a complex journey with emotional, cognitive, physical, spiritual and social aspects.   A single strategy approach doesn't go very far in this journey.  

Diagnostic labels can be deeply misleading to both survivors and clinicians, by riveting attention to a narrow set of issues that deserve consideration but only in the context of a much larger journey.

My core training and the primary tools that I use for interaction with clients come from attachment-based psychotherapy, neuroscience and expressive therapies.   Responding to the insights above, for the past decade I've also invested a great deal of effort and time in wellness and integrative health, including nutritional psychology, to augment my core training.   Goals that I ponder constantly in work with clients and in my writing are sustainability and attaining total wellness.  

Trauma integration takes time, and requires in-depth investigation of all root causes. Some trauma survivors suffer from lingering underlying infections that show up in the form of autoimmune and inflammation disorders, psychological conditions such as depression, anxiety, OCD, self-regulation problems, attention or sensory difficulties, or chronic ill health. Unless we address these, no trauma treatment will help for long. In this case the mental health symptoms will not improve without addressing underlying root causes. 

We also know that trauma affects the body and can be an instigator of chronic fatigue, IBS and IBD, chronic nerve and muscle pain, and other conditions.  In this case, such conditions will not improve without addressing the underlying trauma.

By the time they come to me, many clients have tried a variety of modalities with several therapists.   Typically they report temporary good results with one modality or another, that eventually proved unsustainable.  For such clients, targeting all aspects of wellness is essential.  Working in a linear way, one modality after another is unlikely to assist them.  A comprehensive perspective is essential for progress.  

 

That doesn't mean we have to deal with everything at once.  But we do need to plan strategy in a way that considers all-aspects of wellness.   These include: 

(1) Emotional – dealing with first-line emotions such as fear and anger and second-line emotions such as shame, guilt, and pain.  An important goal is to expand ability to endure pain and enhance joy;

(2) Cognitive – the way we process thoughts, how we focus attention and retention of information; 

(3) Physical – the way we manage nutrition and nourishment, sleep, movement and exercise;
(4) Spiritual – our worldview - the framework we use to find meaning and purpose in life;

(5) Social –  relationships with spouses, family, friends, colleagues, and strangers.

Expressive Trauma Integration™ (ETI) approach is based on over a decade of research and clinical work with traumatized populations in the Middle East, United States, Africa, and Asia by Dr. Odelya Gertel Kraybill, an integrative trauma psychotherapist, and consultant.  A modular approach suitable for both psychosocial and trauma therapy interventions, ETI is an integrative framework that draws on recent research and practice from the fields of neuroscience, attachment and developmental psychology, expressive therapies, experiential and body-oriented therapies, cognitive processing, behavioral modifications, mindfulness, and nutritional psychology. 

 

ETI has been field-tested and revised in trainings provided to social and community workers, therapists, first responders, and students, in Lesotho, S. Africa, S. Korea, Philippines, US, Canada, Israel, Japan and China. The ETI training model was formally accepted by the Philippines Department of Health in 2014 as one of their key modules of response to communal trauma after crisis. 


ETI Pillars 
Psychoeducation about trauma is education about the cognitive, physical, emotional, spiritual and social effects of trauma on survivors and families (individual trauma) and communities (communal trauma).

The Six Stage Trauma Integration Roadmap provides a clear conceptual framework for understanding and responding to trauma.  The ETI approach helps survivors describe their experience in stages of: 1-Routine, 2-Event, 3-Withdrawal, 4-Awareness, 5-Action, 6-Integration.  Survivors locate what they are experiencing on this map and in the safety of a therapeutic setting, use it to guide exploration of further steps towards trauma integration.

Attunement Based Psychotherapy.  Attunement is a nonverbal process of being with another person in a way that attends fully and responsively to that person. A key aspect of attunement is that it is a joint activity, experienced in interaction with a caregiver.  In therapy, the therapist attunes to the client with a goal to become a “co-regulator” with the client's responses. Over time, the client is able to transfer the sense of being co-regulated, to self-regulation outside of the therapy room, in everyday life. Through attuned relationship, clients lean to expand their capacity to endure the pain and loss of trauma and its aftermath.

Self-Regulation refers to the ability to maintain control over one's sensory reactions and expand one's capacity to respond emotionally and cognitively.  Studies show that traumatic memories are stored in locations in the brain that can’t be accessed by cognitive approaches.   Engagement with these memories must come via lower parts of the brain.

 

Whereas talk-based and cognitive approaches work top-down and enlist higher parts of the brain in calming the more primordial functions of the lower brain, nonverbal approaches work bottom-upwards.  They engage directly with the lower parts of the brain and expand from there until the client is ready to engage in top-down narrative processing and trauma integration. 


In the ETI approach, we use tools and techniques that facilitate body-awareness (grounding and embodiment), improve sensory and bilateral integration, strengthen the vagal tone, and promote neuroplasticity (reorganization and formation of brain responses). 

Individualized Sustainability Plan (ISP).  Setbacks are common for trauma survivors, often after encouraging progress.  An ISP is an individual plan designed to assist the continuity of progress. It contains techniques and practices tailored to a client's unique situation and preferences and addresses all aspects of wellness, including emotional, cognitive, physical, spiritual, and social aspects.  

 

ISPs draw from various practices demonstrated to be effective in mitigating trauma symptoms and enhancing the capacity to experience joy.  These include mindfulness and self-compassion practices, sensory and bilateral integration, sport and movement, (top-down) cognitive reframing and behavioral modifications, expressive arts, brain-training, neurofeedback, diet, and nutrition.

The Trauma-Informed Practices and Expressive Arts Therapy Institute is dedicated to providing professional education that promotes greater understanding of expressive arts therapy and trauma-informed practices. The Institute offers live trainings (webinar and real time), courses and workshops and distance learning courses for mental health professionals and master's and doctoral students; these learning opportunities are also open to arts-based facilitators and practitioners. Courses emphasize approaches that are based on best practices, integrative methods and current research in expressive arts and trauma-informed care, and complement mind-body, somatic, sensory integration and neurobiology-informed methods of trauma intervention. We believe that expressive arts therapy [the integrated use of multiple arts forms] in particular is essential to many individuals' trauma reparation and is an important part of that process. Webinar for Parents of Children With Neuropsychiatric Symptoms  

 

Parents of children with neuropsychiatric symptoms struggle with how to help them attain self-regulation and overall well-being. One of the most useful steps parents can take is to provide their children with an understanding of how the brain and nervous system work. 

 

This type of information is defined as psychoeducation. Age-appropriate psychoeducation helps children regain a sense of control when they experience symptoms that make them feel they are at the mercy of emotions they can’t control that are rooted in a hyper nervous system.

 

This webinar will provide parents and caregivers with information essential to understanding how stress affects children with neuropsychiatric symptoms and their families, and describe a roadmap of six stages for moving beyond the cyclic effects of nervous system hyper-activation. 

 

Numerous studies and reports confirm the value of psychoeducation, on clients who suffer from an over-active nervous system. Similarly, a grasp of this key information about how the brain reacts to stress can help children, parents, and caregivers make sense of reactions that otherwise seem overwhelming.

Expressive Trauma Integration (ETI) - An Intervention Framework

I have spent over a decade researching trauma.  My sources have been writings of pioneers in the field, fellow practitioners reflecting on their work, my own life experiences as a survivor of complex trauma, and the experiences of clients and people I have worked with.  

From this has emerged a conceptual framework - I call it Expressive Trauma Integration or ETI – that draws on insights from all the above.  Since first working with it in 2010, I have formally studied the effectiveness of the ETI model with humanitarian aid workers in the southern Africa country of Lesotho (in individual intervention), the Philippines (group intervention conducted by a national agency).  I've also experienced and informally observed its effectiveness in my training and practice in the Philippines, China, Japan, Korea, and the United States.

Central in my thinking in developing the ETI approach has been the conviction that trauma is complex and effects different aspects of our being.  Therefore response to trauma needs to be multidisciplinary. I organize what I consider to be the essential elements of such a response around three pillars: Psychoeducation, Experiential Methods and Individualized Sustainability Plan (ISP).

In this post, I share the six steps towards trauma integration that form the heart of the ETI approach. I hope you find it useful with clients in therapy who are trauma survivors, and training and support of human service practitioners who are working with traumatized clients or in high-stress situations.  With modifications for the level of exposure and the nature of interactional space available, the model fits a variety of needs.

(1) Grounding.  Establishing safety and a sense of stability or grounding in the body and in relationship to surroundings (especially vis a vis other people and safety from physical harm) is an essential starting point.

(2) Psychoeducation about trauma.  I.e: education about the biological, emotional and physical effects of trauma on survivors and families (individual trauma) and communities (communal trauma). ETI experiential psychoeducation uses the Triune Brain model and the Six Stage Trauma Integration Roadmap to explain what is happening in the brain during trauma and what are the expected responses to trauma.

(3) Individualized Sustainability Plan (ISP) is set of techniques and practices designed to provide ongoing cognitive, physical, emotional and spiritual maintenance for clients who are facing adversities. Without a clear plan it is very hard for trauma survivors to sustain themselves longterm.  ISPs draw from various practices demonstrated to be effective in mitigating trauma symptoms. These include: mindfulness and self-compassion practices, sensory and bilateral integration, cognitive processing, behavioral modifications, stress management, expressive arts, diet, and nutrition.

(4) Connecting with Resources.  Connecting to inner resources and/or safe space that existed before trauma is important to sustain the client. If there is none, help build it, using Imaginal Space.  

(5) Taking Action.  The Six Stage Trauma Integration Roadmap (see image below) provides a conceptual framework  (1-Routine, 2-Event, 3-Withdrawal, 4-Awareness, 5-Action, 6-Integration) for exploring steps that could be taken in those stages where the survivor feels frozen.  Imaginal Space serves as a powerful tool for interacting with the possible steps.  

This Roadmap should not be applied as a linear model.  Participants may move from one stage to another, skipping stages and moving back and forth.  Any number of triggers, stressful times, even exciting times, can lead a survivor to any of the stages, both “forwards” and “backwards.”  

The Roadmap aims to provide information and action methods that will enable survivors to locate where they are in their trauma response journey, where they are headed, what challenges they face, and what strengths they possess as they seek integration of their traumatic experiences.  

(6) Trauma Integration – The survivor is supported in reflecting on experiences and insights in the previous stages, with a goal of building a life narrative that incorporates both the trauma experience and personal strengths that enabled surviving it.

Trauma integration is not once-and-done, nor is it linear. It is on-going and sometimes cyclic. But achieving a sense of integration – even just once - establishes the possibility of “breaking the script” of old responses to a traumatic event. Even a single experience of integration opens the door to more new responses. Gradually, experience with new, integrated responses accumulates and the rewards are felt emotionally, cognitively, physically and spiritually.

I think it important to recognize that there are many trauma therapy approaches available, each with strengths and weaknesses. Therapists should not be too attached to any one approach. It is better to encourage clients to listen to and respect their own inner response to whatever approach is being used than for them to blindly accept the guidance of some “perfect model”.

I encourage both trauma survivors and therapists to search for their personal inner compass and follow it to find the path right for them in working for trauma integration.
